If someone is working, pay check stubs for that person's job(s). We'll be asking how much each person makes at their job, how often they get paid, and how many hours they work.
Information about the expenses you pay. For example: How much you pay each month for housing (things like rent, lot rent, mortgage, property taxes, or homeowner's insurance). Which utilities you pay each month (things like electricity, gas, phone, water, and trash removal). If you pay for someone's care (for example, day care for your children), how much you pay and who provides the care. How much you pay for child support each month. Information about your assets. If you're applying for SNAP, we'll ask about the total amount of money you have at home and in your checking accounts and savings accounts.

Keep in mind that if you don't have a way to get the information we've asked for, an application site worker may be able to help you get it.
Here is some more information about some of the people who can help you fill out your medical assistance application: Authorized Representatives must be given permission to act on your behalf and be over the age of 18. Secondary account holders can do everything the head of household can except add other secondary account holders. This person must be a part of the household and be over 18 years of age. Someone who has the legal right to act on your behalf may be asked to provide documentation to prove this.

For CCCAP applications, Counties may require cooperation with Child Support Services for all children in the household, even if not all children are requesting child care.
