Setting Up Your Account


For additional help with Medical Assistance Applications, please call the Health First Colorado Member Contact Center at 1-800-221-3943.

For additional help with requests for Supplemental Nutrition Assistance Program (SNAP) or cash assistance, please call 1-800-536-5298.

Some information about the head of household (primary contact) is required to create an account. The information you enter must match our records.

Required information includes a Social Security Number or State ID or Client ID AND birth date AND Case Number as detailed below:

Step 1

  • ✓ Enter one (1) of the following:

      1. Social Security Number – When entering a Social Security Number, do not use dashes. (Example: 123456789)

      2. State ID – Your State ID can be found on your medical card if you have one. It starts with a letter and is followed by six (6) numbers. (Example: A123456)

      3. Client ID – Your Client ID appears on your benefit letters and has seven (7) numbers. (Example: 1234567)

  • ✓ Enter the Date of Birth for the head of household (primary contact) on the account.

If you do not have a Social Security Number and you don’t know your State ID or Client ID, please contact your local county office for assistance.

Step 2
  • ✓ Enter the Case Number that appears on your benefits letters. Your Case Number begins with “1B” and is followed by a combination of five (5) letters and numbers (Example: 1B*****). If you do not know your Case Number, please contact your local county office for assistance.
